Categorisation and micro-rationing: access to care in a French emergency department

Article Abstract:

This article discusses the categorization of patients by medical staff in a French emergency department. The author examines the decision-making process of the medical personnel regarding who is eligible for care and concludes the emergency department staff takes into consideration clinical, organizational, moral, and social factors, leading to micro-rationing of health care.

On penalties and the Patient's Charter: centralism v. de-centralised governance in the NHS

Article Abstract:

Centralized and de-centralized management of the British National Health Service are comapred. The de-centrlized method of penalty clauses in 'Patient's Charter' guarantees of waiting times is found to be significantly constrained by continuing central direction. Moreover, informal resistance startegies continue to limit the impact of decentralized contractual governance.

Introduction: a sociological perspective on rationing: power, rhetoric and situated practices

This article discusses health care rationing practices. The authors, exploring power relations, economics, and social practices within the health care system, argue the allocation of limited health care resources can be used to support certain interests against others and maintain rationing is a socially constructed reality.
