Improving Hospital Accounting Using a Case-Mix Management System

Article Abstract:

The financial issues are a constant problem in delivering high-quality patient care in Canadian hospitals. There is no effective method of measuring cost implications of patient care used, at present. Kingston General Hospital evaluated several systems in use in Canadian and American hospitals. Finding them all insufficient, they worked to develop their own. Typically, there is no measurement of cost per unit of output. Cost per patient day, per cent occupancy and numbers of examinations have been used, but not effectively. None take account of acutely ill patients. A cost accounting system could be more useful. A medical management group reporting system would recognize diagnosis related groups (DRG's) which provide basis of effectiveness comparison. This plus severity indications, would provide a basis for measurement. At Kingston, four components of the reporting system are identified as product description, measure of product, costing of product and monitoring and controlling product costs. The three elements of costs are identified as hospitalization costs, accommodation costs and clinical care costs. The direct material and direct labor would be easier to quantify than overhead costs. Several benefits of the cost accounting system are listed, including both direct and indirect benefits.
