Patient safety during medication administration: The influence of organizational and individual variables on unsafe work practices and medication errors

Article Abstract:

Structural equation modeling is used to measure organizational climate and to test a model with hypothesized links between climate and unsafe medication administration behaviors. The model provides a reasonable fit to the data with organizational climate, highlighting that deficiencies at the organizational levels affect the psychological well-being resulting in substandard work from hospital employees, which ultimately endangers the patients under their care.

Identification of systems failures in successful paediatric cardiac surgery

Article Abstract:

A direct observation methodology based on a threat and error model is described to identify types and sources of systems failures in paediatric cardiac surgery. It demonstrates the potential benefit of comprehensive data on intra-operative failures, subject to fundamental difficulties with the measurement and classification of errors obtained from observation of events, and a reduction in recurrent failures is recommended to increase patient safety.

Work stress and patient safety: Observer-rated work stressors as predictors of characteristics of safety-related events reported by young nurses

Article Abstract:

A study was conducted to investigate the link between workplace stress and the 'non-singularity' of patient safety-related incidents in the hospital setting, by documenting daily stressful events of young nurses. Job stressors and low job control were shown to be the risk factors for patient safety and the results suggest job redesign to enhance job control and decrease job stressors.
