Article Abstract:

When Alan Hoffberg's company moved, UPS was notified to change the billing and pickup addresses to the new address. Pickups were begun at the new location, but another account was mistakenly given the new address, causing much frustration. Apparently the UPS database system contains no controls for cross-checking an account's billing and pickup addresses. This is an example of how important system analysis, planning, and design are.

Article Abstract:

Once a computer system is operating, any changes that reflect on output should be carefully controlled. The specific changes, the files to be changed, the justification, the effective date, and the authority for the changes should be identified. Changes in the source code of programs should undergo the same controls, either manual or automated. The changes should be documented and reviewed by appropriate designated personnel.
