New ventilatory strategies in acute respiratory failure

Article Abstract:

Researchers are attempting to develop new techniques for avoiding ventilator-induced lung damage in patients in acute respiratory failure. Work is ongoing in determining the optimal positive end-expiratory pressure to prevent overdistention. Extracorporeal life support systems and intravascular oxygenators continue to be used experimentally although improved outcomes have not been demonstrated. Inhaled nitric oxide and tracheal gas insufflation may improve function in mechanically ventilated patients. Guidelines for care are given for each modality.

The emergence of the person-environment interaction in a descriptive study of vigor in heart failure

Article Abstract:

Perceptions of vigor in persons with heart failure can be linked to factors that do not relate to age or disease severity. The results suggest the importance of person-environment interaction. A survey of 54 persons with heart failure revealed that perceived overall health and mental health and participation in an exercise program were associated with vigor. Twelve participants interviewed in depth reported that vigor was influenced by weather, a positive self-image, exercise, a resolve to live, and that physical and mental vigor were linked.
