Nurse practitioners and physician assistants as primary care providers in institutional settings

Article Abstract:

There has been a marked increase in the number of nurse practitioners (NPs) physician assistants (PAs), which could be attributed to the proliferation of managed care. NPs and PAs in HMOs and multispecialty clinics perform routine duties of primary care physicians, to free the latter's time to attend to more complex cases. The study noted that the larger the size of a healthcare facility's managed care, the broader the scope of their duties and the greater their autonomy. The scope of their responsibilities are limited by legal accountability and physician acceptance.

Article Abstract:

United HealthCare, a health maintenance organization based in Minnetonka, MN, has disappointed analysts who thought it would dominate the industry by posting $565 million in losses for the second quarter ending 1998. The company was previously quite profitable to the point that it was considering the acquisition of Louisville, KY, competitor Humana. Observers are fearful that United HealthCare's demise may have a greater impact on the industry rather than on itself because of the market's reaction, noted Health System Change president Paul Ginsburg.
