Richard Doyle, MD, and his team provide clinical practice guidelines through the firm Milliman & Robertson. Guidelines have been provided for ambulatory care, inpatient medical care and inpatient surgical care. Additional guidelines will be published on subacute nursing care, skilled nursing care, pharmaceuticals and workers' compensation in 1996. He believes the firm tries to provide guidelines and not to set policy.

Worker incentives do not have to be financial, and financial incentives have limited usefulness. Many people are motivated by love of their work and a sense of shared purpose. For example, a young library worker can be rewarded nonfinancially by being able to bring home as many books as he or she wants. Managers must be cognizant of such rewards and incorporate them into their compensation plans.
