A doubly difficult birth

Article Abstract:

The delivery of twins at home after a previous delivery by Caesarian section provided a challenging experience for a midwife. The mother insisted on a home birth due to her previous negative hospital experience. However, possible complications included scar rupture. The midwife arranged emergency support in case hospital admission became essential. The delivery was protracted. However cooperation between an independent and community midwife with support from the supervisor of midwives resulted in a safe delivery.

A student midwife recalls her experiences after two years training. Strong memories of her first delivery emphasise the privileged nature of midwifery. She is idealistic about practice and resents any suggestion of compromise. She appreciates the need for sensitivity in dealing with mothers. She indicates the value of encouragement in promoting confidence.

Nurses should be careful when caring for patients suffering from antibiotic-associated diarrhoea caused by Clostridium difficile. Nurses should wash their hands carefully after undertaking procedures and wear protective clothing. The anaerobic bacillus spores are located in soil and in human and animal gut.
