A leading question

Article Abstract:

The case of a 43-year-old man illustrates the necessity of asking all patients whether they use alternative medicines. He was hospitalized with severe abdominal pain, liver abnormalities and anemia. He was also excreting porphyrins in his urine. These compounds are part of the heme molecule, which is present in hemoglobin. It is often a sign of porphyria, and his doctors suspected this was the diagnosis. However, porphyria is usually hereditary and he had no family history of the disease. He finally admitted taking an herbal preparation which was found to be contaminated with lead.

Letting the patient off the hook

Article Abstract:

A 27-year-old man was admitted to a hospital with fever, cough, shortness of breath, and a rash on his buttocks. The rash had appeared during the last week of a month-long vacation in Thailand, where he had sexual relations with several people. His girlfriend also developed a rash. A tropical infection was suspected and a sputum sample contained a hookworm. He was treated with the anti-parasite drug albendazole.
