A service manager model: instituting case management

Article Abstract:

One method of supervising and developing case management strategies is by using a service manager. The case management model seeks to simplify and coordinate the efforts of many hospital departments in treating each patient. A 720-bed hospital used experienced registered nurses as their service managers to oversee case management. Use of this model required restructuring both physically and philosophically but resulted in significant cost savings to the hospital without sacrificing quality patient care.

Building a case management model in a small community hospital

Article Abstract:

Other small hospitals could model and tailor the use of case management strategies based on the preliminary success seen in another small hospital. The case management model seeks to simplify and coordinate the efforts of many hospital departments in treating each patient. A tailored one-page plan for each patient called a critical pathways plan is a method for accomplishing this goal. Nurses are in a unique position to help develop and implement critical pathways plans.

Building a patient-focused care unit

Article Abstract:

A South Dakota hospital developed a patient-focused care model of medical service to improve both patient and provider satisfaction, service efficiency and cost savings. Previous patient care units did not allow easy communication between and often required the patient see 50 or more caregivers. The new model was designed with the participation of hospital personnel.
