Coordinators of care

Article Abstract:

The primary health-care team currently has only a marginal, largely advisory role in care provision for older people in Britain, but a primary health-care-led approach may be the most appropriate to meet the needs of the elderly. The role of the primary health-care team in coordinating care for older people should encompass health promotion, the management of disability and chronic illness, and rehabilitation. A primary health-care-led approach would enable a holistic framework of care to be created by bringing together professionals from health and social services.

Housebound older people are missing out on diabetes care

Article Abstract:

UK district nurses need the support of specialist diabetic professionals in providing support and medical care for diabetic elderly people who are affected by mobility problems. Housebound elderly people may miss out on advances being made in the diabetic medical treatment field, which could have long term consequences for the health of an individual as well as an economic impact on health service provision. A project set up to help elderly people become more involved in their treatment showed changes in diet and support provision can make a difference.

Article Abstract:

Managers in the UK's National Health Services are powerful people who control how health care is organised within government rules. The role of managers has become more important as the health service copes with expanding requirements and limited resources. The existing system has evolved from one where clinicians and health-care professionals took on a management role, leading to conflicting priorities. However, it is now time to review the link between managers and clinicians so that they can form closer partnerships.
