Medicaid proposal may cut physicians' pay; AMA fighting change on 'dual-eligibles.' (elderly persons qualified for both Medicare, Medicaid)

Article Abstract:

The AMA opposes a Senate Medicaid bill which would let states prevent physicians from charging to Medicare part of the costs of treating poor senior citizens who are eligible for both health care programs. Several appeals courts have ruled that states must provide full Medicare charges for cost-sharing. However, guidelines by the Health Care Financing Administration give states the option of paying only the difference if physicians' payments would be lower than the Medicaid rate for that state.

Payment reform - a work in progress

Article Abstract:

The relative value scale payment systems (RBRVS) for Medicare assigns monetary values to medical services based on resources used to provide services. RBRVS consists of three components, including physician work value and professional liability insurance value. Reimbursement rates are calculated using geographically modified relative values for local medical costs. The Health Care Financing Administration establishes relative values for new and redefined services each year.
