Nitric oxide synthase activity in infantile hypertrophic pyloric stenosis

Article Abstract:

Lack of nitric oxide synthase in pyloric tissue may be responsible for the pylorospasm causing pyloric stenosis in infants. Nitric oxide is an endothelium relaxing factor and is important in neurotransmission. Babies with pyloric stenosis have a defect in pyloric relaxation which leads to gastric-outlet obstruction. Both nitric oxide synthase and NADPH diaphorase have identical activity in the brain and peripheral tissues. Nerve fibres in the enlarged circular musculature of these babies' small intestine were enlarged and distorted and had no diaphorase activity. NADPH diaphorase activity was at normal levels in the myenteric plexus and longitudinal muscle layer. Babies without pyloric stenosis had normal enzyme activity levels.

Predictors of outcome in severe, asymptomatic aortic stenosis

Article Abstract:

Valvular calcification and increasing aortic-jet velocity are associated with poor prognoses in patients with aortic valve stenosis without symptoms. Results from a prospective study of 128 men and women revealed that 25% of those with mild aortic-valve calcification died or required surgery during four years, while 80% of those with moderate or greater calcification died or had valve surgery.
