PPRC calls for one-two punch in health system reform

Article Abstract:

The Physician Payment Review Commission proposes that elements from both rate-setting and managed competition health care models be employed in health care reform. For example, a rate-setting system could be used to allow managed care organizations such as HMOs to negotiate their rates with physicians and hospitals. Alternatively, local boards could be allowed to aid in insurance rate-setting; a board would have to approve of an insurance company's plan in order for it to go forward. Several other scenarios are presented in which managed competition and rate-setting could combine in efficient health care delivery.

Oregon in 'Catch-22' spot: state can't get answers on how to change Medicaid reform plan

Article Abstract:

The Health and Human Services Department has created an impasse by ruling that Oregon's new Medicare plan violates a new law protecting the disabled by ranking the cost effectiveness of 709 medical procedures. The plan won high marks for its innovativeness but was condemned for using quality of life criteria to establish a list of treatment priorities. State officials say that the federal ruling is a Catch-22 since all medical procedure are performed with the express aim of improving a person's well-being.

Retiring reformer

Article Abstract:

John Kitzhaber is retiring from Oregon state politics after 14 years. Kitzhaber designed the 'Oregon experiment', the state's rationed health care system. The 'Oregon experiment' was the first US large-scale reform program in use and focused national attention on the issue. Kitzhaber is currently working on a health care cost containment program to present to state legislators in Mar 1993.
