Report: lack of 'safety culture' at NASA contributed to Columbia Disaster

Article Abstract:

The reasons behind the February 1, 2003 Columbia space shuttle disaster are investigated. The investigation committee, although believes foam strike on the shuttle's wing to be the main cause for the shuttle disintegration, blames NASA for lack of organizational safety culture that ensures crises management.

The year 2001 has reported 5,300 workplace deaths compared with more than 52,000 in the home and community. So, with the increased growth of off-the-job injuries, employers should see to it that their employees carry the safety and security measures practiced at the workplace to their home and community
