Article Abstract:

Dumfries and Galloway Community Health NHS Trust in the UK undertook an audit to examine the prevalence of pressure sores in its hospitals and community services. The audit also gathered data on severity of sores, body areas affected and care regimes. The audit revealed a rate of 15.8% in the community hospitals, a higher percentage than the UK government's estimated prevalence of 6.7%, which it wants reduced by 5-10% annually. The audit confirmed previous study findings that the 70-plus age group is most vulnerable to pressure sores, with 87% of the total being in this age group.

Changing the records: An audit at one teaching hospital revealed major shortcomings in the documentation of pressure sore risk

Article Abstract:

Pressure sores are a problem to the health service and to the patient, and the consequences can be minimized by appropriate documentation and treatments. Nurses often do not document care accurately and the lack of documentation of pressure sore risk was recorded in a survey at a teaching hospital. The issue of a more structured care plan led to improved documentation standards.
