Lessons from the wreck of the Exxon Valdez: the need for imagination, empathy, and courage

Article Abstract:

There are two alternative approaches to pinpointing the cause or causes of large-scale industrial accidents. One is legal analysis, which seeks to identify the mechanical failure or human error preceding the accident. The other is technical/social analysis, which looks at the mitigating complexity that made the initial sudden accident and the slow response to it. The wreck of the Exxon Valdez is examined to determine what might have been done at all six levels of managerial responsibility so the accident could have been avoided. Specific changes at all levels are recommended.
