Accuracy of medical records in hip fracture

Article Abstract:

Many medical records may have inaccurate data that could have a significant impact on administrative databases. Researchers compared information coded on the hospital facesheet with the information in the medical chart of 343 patients admitted to a hospital with hip fracture. In 17% of the cases, a complication written in the chart was not coded on the facesheet. In 12% of the cases, excess diagnostic coding was found on the facesheet.

Contribution of individual items to the performance of the Norton pressure ulcer prediction scale

Article Abstract:

The Norton pressure ulcer prediction scale is a reliable assessment model for indicating bedsore risk in hospitalized patients. The model measures patient mobility, activity, mental and physical condition and incontinence, and rates risk on a four-point scale. From among these indicators, the primary factors for bedsore risk are patient mobility and activity.
