Capitated risk-bearing managed care systems could improve end-of-life care

Article Abstract:

Managed care organizations could provide effective end-of-life care if they had a financial incentive to do so. They seldom provide these services because critically ill patients incur large health care costs. However, hospice programs have demonstrated that coordinated care financed by a capitated system is feasible. Managed care organizations specialize in providing coordinated care and interdisciplinary teams and services, while focusing on cost efficiency.

Influence of patient preferences and local health system characteristics on the place of death

Article Abstract:

The location of death for terminally ill patients is not determined by patient preference. In studies comparing hospital to non-hospital mortality rates, findings indicate that location of death is affected by structural characteristics and protocols of the patient's local health care system. Incorporating patient preferences into health care reform may require reorganization of provider routines and resource availability.

Article Abstract:

Many elderly people have a serious chronic illness that will eventually kill them and that they must live with for several years. Examples include cancer, dementia, obstructive lung disease, heart disease, liver disease, and kidney failure. The US health care system does ont serve these patients as well as it could.
