Falling through the cracks: challenges and opportunities for improving transitional care for persons with continuous complex care needs

Article Abstract:

An overview is presented on elderly patients who require care from a wide variety of health professionals in different settings. Required elements for successful care transitions are examined, and the roles of information technology and national policy are discussed.
