Geriatric nurse practitioners in long-term care: demonstration of effectiveness in managed care

Article Abstract:

The use of a geriatric nurse practitioner (GNP) can help reduce the costs of caring for elderly Medicare patients in long-term care facilities. Researchers compared the costs of providing a GNP in addition to a physician with the cost of a physician alone in a study of 1,077 elderly HMO patients in 45 facilities. Acute care and emergency department costs were substantially lower in the GNP group, which saved $72 per patient per month overall compared to a loss of $197 in the other group.

Essential components of geriatric care provided through health maintenance organizations

Article Abstract:

HMOs need to provide special services to their elderly Medicare members. They need to identify members at high risk for serious health problems, provide a case management program, educate their primary care physicians in geriatrics, provide geriatrics experts, provide self-care training for patients and coordinate social services. They should provide wellness programs to support the aging process and make their data available to providers.

A two-year follow-up of geriatric consults in the emergency department

Article Abstract:

Older people who visit an emergency department may need a multidisciplinary geriatric consult as well as close follow-up after discharge. Researchers followed the progress of 326 older patients who were examined by a geriatric consult team (GCT) in the emergency department of a hospital. Sixty-four percent were admitted to the hospital and over a two-year period after discharge, 33.7% had died and 52% had been institutionalized.
