Predicting hospitalization and functional decline in older health plan enrollees: are administrative data as accurate as self-report?

Article Abstract:

An index that is derived from administrative data seems to be as accurate as one derived from patient reports in predicting hospitalization and functional decline in older members of an HMO. The two indexes were compared in a study of 2,174 randomly selected HMO members 65 years and older. A ROC curve program showed that the indexes were similarly effective and the index derived from institutional data would be free from bias and cost less.

Preparing patients and caregivers to participate in care delivered across settings: The care transitions intervention

Article Abstract:

A study was conducted to test whether an intervention designed to encourage older patients and their caregivers to assert a more active role during care transitions can reduce rehospitalization rates. Intervention patients reported high levels of confidence in obtaining essential information for managing their condition, communicating with members of the healthcare team and understanding the medication regimen.
