Interviews with a wide variety of health care executives, analysts, and policy makers about the future of health care revealed several areas of agreement and disagreement. Almost all agreed that the future will bring a single-payer system, perhaps in less than 10 years. There was also agreement that the growing numbers of uninsured middle-class Americans will lead to a political backlash. The role of physicians in the future was more uncertain, with some feeling that they will again become the focus of health care delivery, while others thought that they will become less relevant.

Challenge of poverty: advice for advocates of managed competition
Article Abstract:
Managed health care competition presents a number of special problems regarding care for the poor. The new system must control and monitor utilization while providing a comfortable, dignified atmosphere for treating the underserved. For example, the new system will have to discourage poor people from using the emergency room as their main provider. In addition, provider organizations must become skilled in competing to treat the poor. Several other challenges in treating the poor under managed competition are discussed.

Nonprofit health care organization boards have distinct and necessary responsibilities, chief among them a committment to ensuring board accountability. Board members should actively discuss board duties and performance, taking time to assess actual or potential failings or concerns. It is crucial that boards remain cohesive units, allowing all members to air grievances and make suggestions and never allowing their essential visions to become compromised by the covert actions of disgruntled members.
