Home care's venture into nationwide HMO contracting has resulted in heavy losses and management troubles, contrary to predictions that it would proliferate and merge into a few big corporate players. For instance, Apria Healthcare Group of Costa Mesa, CA, which ventured into national contracting, has incurred a net loss of $34.5 million in the last three quarters of 1997. It has also experienced management troubles such as the difficulty to collect the extra administrative expenses from insurers and to get the referrals promised by insurers.

Hospitals can also benefit by offering related services including injured worker treatment, employee assistance programs and substance abuse treatment. The US national home care sector is undergoing suffering from tumultuous times that have seen an aggressive crusade by Medicare designed to weed out fraud and cut costs. The situation is the very opposite of what industry observers predicted, that the current prevalence of small home care agencies will be replaced by a consolidation movement that will leave only a handful of big corporate players.
