Training and procedural failures raise serious safety questions for Eurotunnel
Article Abstract:
The Channel Tunnel Safety Authority's (CTSA) investigation into the 1996 Tunnel fire indicated Eurotunnel management failed to fix previously-noted emergency systems problems. The report also cited many procedural errors in handling that emergency, as well as problems inherent in the evacuation strategy. CTSA attributed the problems to weaknesses in staff training and raised safety issues concerning the use of heavy goods vehicles in the Tunnel.

The Associated Octel fire
Article Abstract:
Associated Octel Company Ltd had a major fire at a United Kingdom site in 1994 due to inadequate risk assessment and safety management, according to an investigation and report by the HSE. Problems included pump failure, poor plant maintenance and off-site plans. The fire resembles two UK industrial fires in 1992, at companies with similar deficiencies in risk assesement.
