The "cog in the machine" manifesto: the banality and the inevitability of evil
Article Abstract:
Diane Vaughan argued of the inapplicability of the 'cog in the machine' manifesto to the Challenger launch decision which resulted in the death of five astronauts and two civilians. The fallacies of the manifesto were exposed by an alternative umbrella thesis and four sub-theses. These included theories that the Challenger disaster was caused by the lack of a safety measure, that inherent risk was a misleading justification, that risky assessment was the real culprit, that the use of an Orbiter showed the lack of a safety measure and that the launching was not a banal mistake.

Shut that door
Article Abstract:
The UK Door and Shutter Manufacturers' Association (DSMA) supports regulations, soon to become law, concerning the regular inspection and maintenance of work equipment. The new law will make building owners, builders and managers liable for the maintenance of installed equipment, including power-operated shutters and doors. The DSMA recommends that parties responsible for the maintenance of power-operated doors should ensure that they are fitted by a company offering a maintenance service and that they bear a CE mark. They should also keep maintenance records.
