Outbreak of pneumonia in a long-term care facility: antecedent human parinfluenza virus 1 infection may predispose to bacterial pneumonia
Article Abstract:
A study of causes of a 10-day lobar pneumonia outbreak in a chronic care facility for the elderly has been made. For four of the 10 case-patients, routine diagnostic cultures of blood or sputum were used before antibiotics were given. Noninfluenza viral infections may play a role in development some bacterial pneumonias. S. pneumoniae was the source of at least two cases in the 10. Lack of cultures taken before antibiotics were given may have made it difficult to isolate S. pneumoniae in others. Spread of HPIV1 in the facility may have made residents more susceptible to S. pneumoniae and other bacteria. Based on records, only 4.5% of the residents of the facility had been given the pneumococcal vaccination.

The effect of HMO status on the outcomes of home-care after hospitalization in a Medicare population
Article Abstract:
Medicare patients enrolled in health maintenance organizations (HMOs) and those in fee-for-service (FFS) insurance plans may receive home health care of similar quality after hospital discharge. Researchers compared post-discharge care and functional outcomes for 970 patients after hospitalization for acute illness. HMO patients were 1.7 times as likely to be discharged to a nursing home, compared to FFS patients. Among patients discharged to the home with health care assistance, patients performed equally well in activities of daily living, regardless of insurance type.

Creating a teaching geriatric service: ten important lessons
Article Abstract:
The Medical Services at the Weill/Cornell campus of the New York Prebysterian Hospital has created a geriatrics inpatient team and a seventeen bed Acute Care for Elders unit. It has also designed a program to explain the fundamental principles of geriatric care. The ten important aspects that can be learnt considering the pitfalls and potentials that underlie in creation of the program are discussed.
