Medicaid expansions for pregnant women and infants: easing hospitals' uncompensated care burdens?

Article Abstract:

The impact of expanded Medicaid coverage on the uncompensated care provided by hospitals is studied. Medicaid coverage was extended to pregnant women and infants with family incomes up to 185% of the federal poverty level in 1987, to offset the rising uncompensated charity care provided by hospitals. It is shown that Medicaid expansions decreased uncompensated care by about 5.4% in most hospitals, while 28.5% reductions were obtained in hospitals devoted to maternity and infant care.

Case study: the Washington State Health Care Authority

The Washington State Health Care Authority (HCA) implements health status risk adjustment and continues to refine the process by adapting a set of guiding principles that include being practical and feasible, focusing on data that are advantageous to plans, and assuring proper safeguards to protect individuals' rights to privacy on health status information. The groups also focus on refinement, rather than reinvention, and aim for better not for the best.

Health service researchers must be able to respond to changes in healthcare financing, delivery and education by focusing on the core activities of monitoring, interpretation, evaluation and recommendation. Monitoring involves identifying the changes in health care while interpretation means understanding these changes. Evaluation involves an assessment of the social desirability of change followed by policy recommendations.
