Benchmarking: improving outcomes for the congestive heart failure population

Article Abstract:

Benchmarking can improve the quality of health care for congestive heart failure (CHF) patients. A multidisciplinary hospital team closely examined clinical indicators, methods of care, and quality controls to determine the five medical processes which needed improvement. Each of five project teams identified process goals for reduced or improved mortality rates in CHF patients, and the goals were met. Benchmarking helped lower costs and ensure quality improvement.

Congestive heart failure: a continuum of care

Article Abstract:

Patients with congestive heart failure need access to appropriate services after discharge from the hospital. Advocate Health Care in Illinois set up such a program. Continuous care, communication, and patient education are crucial steps in providing seamless services beyond the hospital. Finding a supportive physician and setting up drug protocols for each patient were also important. Close contact with patient families was provided throughout the duration.

Quality forum: benchmarking and congestive heart failure - a study in improvement of health care processes

Article Abstract:

Benchmarking, the use of performance information to improve practice, can also be applied to health care. A Pennsylvania hospital implemented the changes recommended for patients with congestive heart failure (CHF) because of longer hospital stays and high readmission rates. Preventing delays in the nursing treatment of stable CHF patients, patient education, and detailed discharge planning resulted in a perceived change for the better.
