Diagnostic tests for hepatitis C: where are we now?

Article Abstract:

It may be advisable to use different types of tests for diagnosing and monitoring hepatitis C viral (HCV) infections depending on the patient's condition. An enzyme test called enzyme-linked immunosorbent assay (ELISA) may be appropriate in patients with non-critical hepatitis symptoms. Doctors should then perform a baseline branched-chain DNA (bDNA) test if these patients are to receive treatment and continue monitoring the patient with this test throughout treatment. The only test available sensitive enough to verify a successful treatment is reverse transcription polymerase chain reaction (RT-PCR). The use of RT-PCR is also appropriate for patients with critical hepatitis symptoms. Doctors should perform liver biopsies only on patients with long-term hepatitis to monitor disease progression and to predict response to treatment.

Heterosexual transmission of HIV in Haiti

Article Abstract:

Professional counseling and free condoms can considerably decrease HIV transmission among heterosexual couples in Haiti. Over 90% of Haiti's AIDs patients contract HIV infection through heterosexual contact. Of 177 couples in which one partner was infected with HIV, 20 uninfected partners became HIV-positive. Only one uninfected partner among 42 sexually active couples who used condoms became HIV-positive. Fifty-five percent of couples had unprotected sex and had an HIV infection rate of 6.8 per 100 person-years. Factors associated with HIV transmission included syphilis, genital ulcers and vaginal and penile discharge.
