Article Abstract:

A software product risk assessment package has been designed by a UK engineering company. It can make or review assessments at any design review stage. The user has to: identify and input hazards present and non-present; input the nature and level of risks for each identified hazard at each stage and specify the action which will need to be taken to eliminate the hazard. Sixteen hazard types have been identified and 'expert knowledge' will be included in the next version. The package will help comply with safety aspects of risk assessment and will help against negligence claims.

Article Abstract:

Channel tunnel transport operators are desperately looking for proof that their open-lattice-design wagons were not responsible for the spread of a fire that swept through the tunnel in Nov 1996. The fire was almost certainly caused by inadequate wagon designs, according to fire officers who advised on the use of fully-enclosed carriages. Safety mechanisms can only be evaluated effectively when products are being designed.
