Ring around the diagnosis

Article Abstract:

A case-study of a 71-year-old retired schoolteacher from rural Ohio with a two-week history of malaise, fever, anorexia, chills and sweats is discussed with reference to its clinical diagnosis. Q fever was found to be most likely diagnosis for this patient's given his exposure to farm animals, and the patient's clinical status improved dramatically within days after the initiation of doxycycline and was discharged home to finish a three-week course of this drug.

A jaundiced eye

Article Abstract:

A case study of a 27-year old man with cytomegalovirus (CMV) infection illustrates the presentation of CMV infection as a severe multisystem disorder, though his initial presentation to the physician was confusing and misleading. It demonstrates that to make the correct diagnosis, the experienced clinician must occasionally view uncharacteristic findings with suspicion or through a 'jaundiced eye' to avoid erroneously ruling out the correct diagnosis.

Article Abstract:

A clinical case study of urinary urgency and fever developed in a 55-year-old, bedridden woman with multiple sclerosis, a long-term indwelling Foley catheter, and multiple prior urinary tract infections is presented. The patient's urinary urgency and hematuria probably resulted from methotrexate-induced leukopenia, which confers a predisposition to infection, and thrombocytopenia, respectively.
