The Value of Targeted Case Management During Transitional Care

Article Abstract:

Medical personnel must design programs that provide continuous care for discharged hospital patients with chronic diseases. Patients who are hospitalized once are likely to be hospitalized again. A 1999 study showed that the use of advanced practice nurses to provide follow-up care and home visits after discharge reduced the re-hospitalization rate in a group of elderly hospital patients. This one-month intervention appeared to lower re-hospitalization rates for several months afterward. This indicates that the patients were better able to cope with their medical problems.

Long-term Effects of Nurse Home Visitation on Children's Criminal and Antisocial Behavior: 15-year follow-up of a randomized controlled trial

Article Abstract:

Children born to mothers who received at-home nursing visits before and after birth may be less likely to run away from home, commit crimes, and smoke and drink at age 15. Researchers followed 400 children born to mothers, some of whom had had visits by a nurse around the time of pregnancy. Children of mothers who had home visits had 20% as many convictions as other children, fewer lifetime sex partners, and drank less alcohol.

Home care

Article Abstract:

Many health care services can be provided in the patient's home. Unfortunately, the 1997 Balanced Budget Act reduced Medicare spending on home care services. Most funding is provided only for patients who have been discharged from a hospital. Most home health care is provided by unpaid family members, which puts a severe strain on the family's financial and psychological resources.
