The chest-pain unit - ready for prime time?

Article Abstract:

Many hospitals may have difficulty maintaining a chest-pain unit (CPU). A CPU is an area in the emergency department specifically designed to monitor patients with chest pain. Usually these patients are admitted to the hospital, but up to 40% of patients with chest pain do not have serious heart disease. A 1998 study found that monitoring patients in the CPU was substantially cheaper than admitting them. However, the study was done at the Mayo Clinic, and many hospitals may not have the resources that this prestigious hospital has.

Amiodarone or an implantable cardioverter-defibrillator for congestive heart failure

Article Abstract:

A study to examine patients suffering with congestive heart failure (CHF) is proposed to treat them with amiodarone or an implantable cardioverter-defibrillator (ICD) to improve the prognosis in such patients. It was found that in patients with New York Heart Association (NYHA) class II or III CHF and left ventricular ejection fraction (LVEF) of 35 percent or less, amiodarone has no favorable effect on survival, whereas single-lead, shock-only ICD therapy reduces overall mortality by 23 percent.

A 73-year-old male patient with stable coronary artery disease, hypertension, and chronic renal insufficiency was presented with recurrent atrial fibrillation at 80 to 90 beats per minute, as well as shortness of breath and fatigue. The clinical problem, as well as its management with oral amiodarone is discussed.
