Intraosseus infusion

Article Abstract:

When children need quick infusions of substances into their vascular systems, dangerous delays can result from the difficulty of gaining access to a blood vessel. Such situations arise in cardiopulmonary arrest (cessation of function of the lungs and heart), burns, status epilepticus (prolonged seizures that can become life-threatening), and shock. An alternative to blood vessels for infusions is the marrow cavity of long bones, a space that allows substances to enter the general circulation almost immediately. This can only work in children under the age of five, because the marrow's supply of blood vessels becomes less after that age. The technique is not recommended in certain situations, such as when bone disease or fractures are present, and caution should be used if inflammation or an infected burn is present. The technique is described. Complications include problems of placement, leakage of fluid, or clotting of marrow in the needle. Rates of localized infection (cellulitis and abscesses) are lower than rates of infection from indwelling catheters in blood vessels. Osteomyelitis (bone inflammation) is a serious complication, and has been noted to occur in 0.6 percent of 4,270 cases. In summary, intraosseus infusion has several advantages as a method of vascular access. It is currently used on children in crisis, where other approaches to infusion have failed.
