Nursing home characteristics associated with tube feeding in advanced cognitive impairment

Article Abstract:

In a n ursing home study it was found that having a full-time speech therapist on staff, more licensed nurses and fewer nursing assistants were independently associated with greater use of tube feeding in severely cognitively impaired nursing home residents. P otentially modifiable factors in nursing homes that may influence the practice of tube feeding in advanced cognitive impairment include advance directives, staffing ratio s, assessment by a speech therapist, specialized dementia units, and fiscal considerations. It seems limited resources and fiscal incentives motivate use of the intervention, which is inc reasingly shown to be not beneficial. Better terminal care may be given in a unit dedicated to this population.

Decision to hospitalize nursing home residents dying with advanced dementia

Article Abstract:

The factors affecting the decision whether or not to hospitalize nursing home residents with advanced dementia or Alzheimer's disease are discussed. The decision becomes all the more crucial at a time when the dementia patient is dying as it can result in considerable increase in healthcare costs apart from treatment difficulties. Nursing homes provide terminal care to patients and are subject to do-not-hospitalize order, which is applicable in the case of some of the residents.

Factors that improve advanced dementia patients' satisfaction regarding end-of-life care, based on healthcare proxies' scores, are presented.
