Can APNs be independent gatekeepers?
Article Abstract:
Presented are affirmative and negative sides of the debate on whether nurse practitioners are qualified to make gatekeeper decisions in the administration of primary medical care in the reformed health care system. Both sides agree that the role of nurses will expand. Also, a shortage of trained primary care personnel is expected. Many advanced practice nurses feel qualified to exercise an independent role. However, most physicians claim nurses lack sufficient training and experience to make clinical decisions.

Disaster in Oklahoma City
Article Abstract:
The physician who headed St. Anthony Hospital's response to the bombing of Oklahoma City, OK's Murrah Federal Building recounts his and his colleagues' experiences. He examined patients on the sidewalk as they were delivered and, based upon their injuries, assigned specialists to them. The hospital sent two medical teams to the blast site to provide treatment and additional communication.

Reality check: it's time for nurses to face the future

Nurses' roles are being redefined as changes sweep the healthcare industry, but nurses are fighting hard against that redefinition. Nurses oppose many healthcare system changes because they fear that ineffective systems are being instituted, at the cost of lower care-quality. Nurses must learn to accept the fact that change is necessary for healthcare organizations' survival.
