Hemostatic complications in young patients with essential thrombocythemia

Article Abstract:

Essential thrombocythemia (ET), a disease characterized by alternating episodes of spontaneous bleeding and clot formation, is an acquired disease of the bone marrow. ET may result in enlarged spleen, highly elevated platelet count (a blood clotting factor) and platelet dysfunction. This disease usually occurs in middle age, in the 50's and 60's, but young people who are asymptomatic have also been diagnosed with this disease. This study was designed to describe the disease in young people; medical records of 44 patients under 45 years of age were studied. In 31 patients, ET was discovered incidentally by routine automated blood testing for clotting factors; 13 patients were identified because of symptoms. There were 17 patients (39 percent) who experienced hemorrhage or thrombosis (formation of a clot in a blood vessel). Screening for acquired von Willebrand's disease, a disease characterized by abnormally slow blood clotting, was performed in seven patients; findings were abnormal in five cases. Seven of these abnormalities were minor, while ten patients (23 percent) had serious complications, which included transient ischemic attack, various neurologic complaints, and deep muscle hemorrhage; two patients died. Neither the presence of symptoms nor any single laboratory value was a predictor of clinical course, and treatment with anti-platelet drugs was not helpful. Communion with Colin: a case study of halting a retreat from anxiety

Article Abstract:

A case study of a 16-year-old boy who was referred to a social worker for severe procrastination in completing school work was presented. Although the boy was nearly mute at times, the social worker still expressed her strong need for understanding his emotions. The therapeutic value of social workers, counselors and teachers in finding opportunities to share in students' emotional experiences was discussed. Likewise, the concepts of affect attunement, separation anxiety and social referencing were also analyzed in relation with the case study.
