How the US drug safety system should be changed

Article Abstract:

The present system of drug safety monitoring includes preclinical testing followed by 3 phases of clinical studies. Drug approval is sometimes followed by post-marketing studies and usually only 500 to 3000 patients are studied before marketing, therefore, by definition, adverse events that occur in 1 of 100 patients would be reliably detected, but adverse reactions that occur in 1 in 1000 patients or less commonly might not be detected, even if these reactions are very severe.
