Impact of laboratory-initiated reporting of CD4+ T lymphocytes on U.S. AIDS surveillance

Article Abstract:

States that report CD4+ T cell counts appear to report AIDS cases at an earlier stage of infection. The case definition of AIDS was changed in 1993 to include CD4+ T cell counts less than 200. Many states began reporting CD4+ counts in preparation for the changed definition. A survey of states found that states that reported CD4+ counts also reported more AIDS cases compared to states that did not report CD4+ counts. States with CD4+ reporting also reported AIDS cases at an earlier stage when the disease is amenable to treatment.

Among the 415 men, 52% were classified, 44% has sex with other men, 28% were injection drug users, 22% had a heterosexual risk, and 6% had other exposures. Among the 219 women, 62% were classified, 60% had a heterosexual risk, 35% were injection drug users, and 4% had infection through blood transfusion.

Unreported AIDS-defining opportunistic illnesses in persons reported with HIV-related severe immunosuppression

Article Abstract:

A study of 3,289 patients diagnosed with AIDS after the CDC changed the case definition in 1993 found that 10% had an unreported opportunistic infection. The case definition of AIDS was changed to include severe immunosuppression in the absence of an opportunistic infection.
