Muddling the message? Critics say Medicare study is flawed, but agree pay is too low

Article Abstract:

William Hsiao, a Medicare physician payment expert, has performed a study that indicates that physicians are underpaid for services and overpaid for procedures under Medicare's payment reform system. The study calculated reimbursement based on a service representative of each type of physician's practice. It found, for example, that family physicians would make only $40,000 if Medicare were the only payer. The study, which was criticized for its methodology, concluded that the medical system is distorted by too many specialists and by physicians who order too many procedures.

Medicare's Resource-Based Relative Value Scale (RBRVS) for physician reimbursement was amended to make it more equitable, with the help of recommendations from the American Medical Association. As a result, many medical groups were pleased with the changes. Reactions to the RBRVS from the American Medical Association and almost 20 other national medical associations are presented.
