A prospective study of the efficacy of The Physician Order Form for Life-Sustaining Treatment

Article Abstract:

A prospective study has been carried out on efficacy of a one-page form called The Physician Orders Form for Life-Sustaining Treatment (POLST). Chart record data was used to follow the nursing home residents whose forms called for no resuscitation prospectively. The patients were to be moved to a hospital only if comfort measures were not adequate. No subject received ICU care, CPR or ventilator support. Only 2% were hospitalized to extend life, and 38 of the 180 total subjects died in the year of the study, while 63% had an order for narcotics.

Out-of-hospital death: advance care planning, decedent symptoms, and caregiver burden

Article Abstract:

The end-of-life experiences of elderly descendents dying out of the hospital and their family caregivers in a state in which the vast majority of Medicare deaths occur in the community settings are examined. The perceived symptom distress is high for a subset of decedents, and caregiver strain is common despite the high rates of advance directives and hospice enrollment.

The physician orders for life-sustaining treatment program: Oregon emergency medical technicians' practical experiences and attitudes

Article Abstract:

A study was conducted to evaluate emergency medical technicians' (EMTS) experiences with the physician orders for life-sustaining treatment (POLST) program and learn about attitudes regarding its effectiveness. EMTs find the POLST form useful and they often use it to change treatment decisions for patients.
