Systems of care for older populations of the future
Article Abstract:
Managed care organizations need to develop coordinated systems of care for their elderly Medicare patients. Many elderly people have chronic diseases, such as congestive heart failure or the complications of stroke. Some of the systems that have been developed include acute care for elders (ACE) units, home health care, subacute care units, case management and adult day health care. Assessment tools are needed to identify patients who will require these services.
