Error recovery in a hospital pharmacy

Article Abstract:

A field study was performed in a hospital pharmacy to identify positive and negative influences on the process of detection of and further recovery from initial errors or other failures, thus avoiding negative consequences. The analysis revealed that organizational culture with regard to following procedures needed reinforcement, some procedures could be improved, building in extra checks was worthwhile and that supporting unplanned recovery was essential for problems not covered by procedures.

Patient safety in outpatient surgery: The viewpoint of the healthcare providers

Article Abstract:

A study was conducted to understand the viewpoint of healthcare providers with regard to patient safety in outpatient surgery settings. The results of the structured questionnaire show that, overall, the healthcare providers report high quality of care provided by themselves (98%) and their surgery centre (96%), highlighting the importance of getting input from the healthcare providers regarding quality and safety of care.

Error detection: A study in anaesthesia

Article Abstract:

The existing scientific knowledge on error detection is synthesized, based on studies conducted in laboratory or self reporting and further knowledge is gained through the analysis of a corpus of cases collected in a complex system, anaesthesia. Significant relationships are found between the type of error and error detection mode and between the type of error and training level of anaesthetist who commits the error.
