Healthcare purchasing heavyweights VHA Inc and Premier Health Alliance are caught up in a fierce struggle for dominance at a time when such key healthcare systems as Columbia/HCA Healthcare and Kaiser Permanente are forging their own paths instead of relying on the purchasing alliances. The alliances have been able to increase their clout through deft dealing and mergers. Their usefulness to the big systems, however, may be over.
